Description
Staged Lipedema Surgery With Body Lifts After GLP-1 Weight Loss: 42-Year-Old Woman
Before Treatment
This 42-year-old woman came to board-certified plastic surgeon Dr. John Larson in Newport Beach, California, with lipedema affecting her legs, hips, buttocks, arms, and abdomen. Over the years, three cesarean deliveries had also changed her abdomen. Like many women with lipedema, she found that diet and exercise did little to reduce the disproportionate, painful fat in her legs and arms.
Lipedema is a chronic fat disorder, affecting mostly women, in which abnormal fat builds up symmetrically in the legs, hips, and often the arms. Lipedema fat largely resists diet and exercise, and it often brings pain, heaviness, and swelling that affect daily life. Because it involves several areas of the body, treatment is usually planned in stages.
Her main concerns:
- Lipedema of both legs, from the thighs through the ankles, and the hips and buttocks
- Lipedema of both arms
- Pain and heaviness in her legs and arms
- Swelling and easy bruising in the affected areas
- Limited mobility
- Excess abdominal tissue after three cesarean deliveries
Her Surgical Journey
We treated her lipedema in five stages over about 15 months, combining power-assisted liposuction to remove the lipedema tissue with lifts to remove excess skin.
During her treatment, she began a GLP-1 medication and lost a significant amount of weight. Her body volume dropped substantially, leaving a large amount of loose skin. Her later stages were planned around this change and included extensive skin removal to tighten and reshape the areas affected by her weight loss.
- Stage 1: Full-leg lipedema liposuction with thigh lift (April 2025). Power-assisted liposuction of both legs, hips, and buttocks, combined with a thigh lift to remove excess skin.
- Stage 2: Arm lipedema liposuction with arm lift (July 2025). Power-assisted liposuction of both arms, combined with an arm lift to remove loose skin and contour the upper arms.
- Stage 3: Abdominoplasty with panniculectomy (December 2025). Liposuction of the abdomen with removal of the overhanging skin and tissue and tightening of the abdominal wall.
- Stage 4: Hip lift and superior buttock lift (April 2026). Additional power-assisted liposuction of the legs, hips, and buttocks, combined with a hip lift and an upper buttock lift.
- Stage 5: Inferior buttock lift (July 2026). A lower buttock lift to remove remaining excess skin and refine the transition between the buttocks and thighs.
How She’s Doing Now
She is doing very well. Her photos were taken just over two months after her final stage and about 17 months after her first surgery. Her contours will continue to refine as swelling resolves and her scars mature over the coming months.
- Improvement in pain and heaviness in her legs and arms
- Improvement in swelling
- Improved mobility
- Slimmer, more proportionate legs, hips, and arms
- A flatter, tighter abdomen
- Smoother, tighter contours through the thighs, hips, and buttocks after significant weight loss
Over five stages, her treatment addressed both her lipedema and the loose skin left after major weight loss, giving her a lighter, more comfortable, and more proportionate body.
Common Questions About This Case
Can you have lipedema surgery while taking a GLP-1 medication?
Yes, with careful planning. This patient began a GLP-1 medication during her treatment and lost a significant amount of weight. We adjusted her surgical plan as her body changed, adding skin-removal procedures to address the loose skin her weight loss left behind.
Why did she need so many skin-removal procedures?
Liposuction removes lipedema fat, but it does not remove excess skin. Her substantial weight loss on a GLP-1 medication left loose skin on her thighs, arms, abdomen, hips, and buttocks. Lifts in each area removed that skin and tightened the remaining tissue for a smoother, more natural contour.
Why is lipedema surgery done in stages?
Lipedema often affects several areas of the body. Staging lets us treat each area thoroughly while keeping every operation safe and recovery manageable. This patient’s care was completed in five stages over about 15 months.
Can lipedema surgery be combined with a tummy tuck after C-sections?
Yes. This patient had three cesarean deliveries before her lipedema care. Her abdominal stage combined liposuction with an abdominoplasty and panniculectomy to remove the overhanging tissue and tighten her abdomen.
How long does it take to see final results after body-lift surgery?
This patient’s photos were taken just over two months after her final stage. Swelling continues to resolve and scars continue to fade for many months, so her results will keep improving.
