Lipedema Transformation: 53-Year-Old Woman – Dramatic Progress in Severe Fibrotic Lipedema

Before Treatment

This 53-year-old patient presented with advanced lipedema affecting her arms, abdomen, hips, buttocks, and lower extremities. She had particularly severe fibrotic changes with excess fat and dense tissue hardening in the saddlebag region that caused significant discomfort and functional limitations. Her symptoms included intense pain, heaviness, and easy bruising in the legs, hips, buttocks, and saddlebags. The severe fibrotic tissue in the saddlebag area restricted movement and caused constant discomfort. She also experienced a disproportionate lower body and arms that impacted her silhouette and clothing options, along with reduced mobility, daily limitations, and an emotional toll from long-standing symptoms.

Her Surgical Journey

We designed a comprehensive four-stage plan using power-assisted liposuction with targeted fibrosis breakdown and strategic skin-tightening procedures to achieve smooth, balanced contours:

  • Stage 1: Thorough circumferential power-assisted liposuction of bilateral lower extremities and hips, with extended time dedicated to breaking up the dense fibrotic tissue in the saddlebag area → Removed 8 liters
  • Stage 2: Circumferential liposuction of bilateral upper extremities + extended brachioplasty (arm lift) → Removed 2 liters (1 liter per arm)
  • Upcoming Stage 3: Additional liposuction of bilateral lower legs with extended thigh and calf lifts
  • Final Stage 4: Lipoabdominoplasty with circumferential extension around the hips and buttocks to address skin laxity created by removal of fibrotic saddlebag tissue

Total lipedema fat removed so far: 10 liters (additional volume planned in future stages)

How She’s Doing Now

She recovered beautifully from her first-stage lower body procedure and is now recovering very well from her recent arm surgery. She is thrilled with her progress so far, with a marked reduction in lower body pain and heaviness. Her legs and hips are more proportionate, with significantly improved mobility. Her arms are beautifully contoured following her recent second operation (arm photos coming soon). She has renewed energy, confidence, and excitement as she looks forward to completing her transformation.

This carefully staged approach has already delivered life-changing pain relief and aesthetic improvement, particularly in her most severely affected saddlebag region. With planned future stages—including extended thigh and calf lifts followed by circumferential lipoabdominoplasty and hip lift—she is on track for even more balanced, comfortable, and proportionate results.
